TFAA

Form Health has devised the Telehealth Functional Abilities Assessment (TFAA) to provide companies direction for specific claims management solutions. Through video conferencing with an individual and the completion of online questionnaires and an in-depth clinical history, the assessor is able to provide the claims manager with up-to-date current medical and functional status. Appropriate recommendations may also be provided in the report to expediate a return to work.

RCA

The Remote Cognitive Assessment (RCA) has been devised to provide companies with an up-to-date review of the individual’s current baseline cognitive function through a battery of CNSVS and MMSE tests.

VOCATIONAL REHABILITATION (VR)

An assessment and case-management service to facilitate sustaining employees working or expedite their return to work. Consultants will explore the reported medical and non-medical challenges from an employee and employer perspective then, in conjunction with all parties, design and implement a programme to achieve the optimum outcome in a timely manner.

PHYSICAL THERAPY AND COGNITIVE BEHAVIOURAL TREATMENT

Remote fast-track solutions for individuals requiring immediate treatment to facilitate a successful return to work. Treatment modalities provided include physiotherapy, acupuncture and CBT delivered through our nationwide database of therapists. We specialise in work-focused therapy whereby treatment plans are specifically designed to target and facilitate a long-term successful return-to-work.

ACTIVE WORK SOLUTION (AWS)

A solution-focused service conducted by professionals trained to a high level in HR. Their independent perspective to a situation enables them to progress and resolve employee/employer conflict situations including both mediation and negotiation strategies to seek out and achieve mutually agreeable resolutions from a commercial standpoint. Outcomes typically include employee return-to-work or settlement agreements.

OCCUPATIONAL HEALTH PHYSICIAN ASSESSMENT (OHP)

A nationwide accredited network of Occupational Health Practitioners across many different specialities in addition to an in-house Company Medical Officer to independently medically review cases.

PRE-EMPLOYMENT SCREENING

Pre-employment medical screening to provide a prospective employee’s health prior to starting in the company. Designed to ensure they are fit for specific work and, where appropriate, detailing any reasonable adjustments that require implementation prior to their engagement.

INDEPENDENT MEDICAL EXAMINATION (IME)

We are continuing to provide, where possible, specialist assessment from a practitioner within our nationwide network of Independent Medical Examiners across many different specialities.

TRANSFERABLE SKILLS ANALYSIS (TSA)

An assessment to determine an individual’s skills and ability to conduct alternative roles. This assessment is typically used within situations where the individual’s previous role no longer exists or they can no longer perform their normal form of employment due to injury or a medical condition. This can be used for claims-assessment purposes or return-to-work planning. Depending on the needs of the situation it could be conducted as a desktop exercise or may include an telephonic assessment with an Occupational Psychologist.

IMMEDIATE NEEDS ASSESSMENT (INA)

An in-depth assessment for individuals who have been involved in serious injuries, detailing their requirements from both a medical and vocational perspective. Designed to successfully assist, support and rehabilitate the employee in their change of circumstances.

MENTAL-HEALTH ASSESSMENT (MHA)

A screening assessment conducted by Chartered Occupational Psychologists to determine whether an individual may be impacted by a mental-health condition and recommend the appropriate course of action to help facilitate a return to work.

DYSLEXIA/DYSPRAXIA/DYSCALCULIA ASSESSMENT

Conducted by a Psychologist to determine whether an individual has any of these conditions and provide practical recommendations on how any challenges identified may be managed in the workplace.

REPORTING, PEER-REVIEW AND ASSOCIATED SERVICE

Provision and full support for medico-legal and all other IME reports including independent peer-review, evidence and records gathering, advice and supply of recommendations.

WORKPLACE ASSESSMENTS FOR VISUALLY IMPAIRED (WAVI)

Specialist workplace assessments for individuals with visual impairments, to identify potential risks and challenges and provide solutions.

SPECIAL WORKPLACE ASSESSMENTS FOR HEARING IMPAIRED (WAHI)

Specialist workplace assessments for individuals with hearing impairments, to identify potential risks and challenges and provide solutions.

WORKPLACE ASSISTIVE TECHNOLOGY (WAT)

Specialist service providing assessment, software, equipment, training and coaching for individuals requiring support with neurodiverse conditions, visual or hearing impairments, or difficulties using a mouse or keyboard.

CORPORATE TRAINING

Various training sessions available including Manual Handling, Mental Health First Aid, Stress Awareness, and Resilience.
